This spring, Simon Fraser University went live on its new Grade Change Request eForm. The newest form for PeopleSoft Campus Solutions, the SFU’s Grade Change Request form enables instructors or department staff to submit a request for a grade change to a student’s academic record.
 
For SFU, the form is accessed through the Faculty Center as a self-service action. Based on the user’s login credentials, the Grade Change Request form automatically presents the instructor with a list of his/her classes and student rosters for the past two semesters, including the current grade associated with each student and whether a Grade Change Request is already in progress. The instructor selects the appropriate student (automatically populating the form with the necessary student data), and enters the revised grade, reason and any supporting comments. Once the form is complete, it routinely routes to a Department Chair for approval and then automatically updates PeopleSoft. Conditional routing options exist if an approver is absent or unavailable. Notifications are automatically triggered for the instructor, advisor, student and others that may need to be informed.
 
With more than 1,000 grade change requests processed each semester, the new eForm is having an immediate impact -- reducing the turnaround time required for processing change requests, providing users 24/7 visibility on the status of each grade change request, and making the process significantly easier for both initiators and approvers. Congratulations, SFU!
